Prescribing considerations
- Initiate and supervise treatment through clinicians experienced in chronic graft-versus-host disease.
- Confirm the licensed age and previous systemic-treatment criteria.
- Review pregnancy status, contraception, hepatic and renal function, and interacting medicines before initiation.
- Strong CYP3A4 inducers and gastric acid suppression can materially reduce exposure.
- Use is not recommended in moderate or severe hepatic impairment without liver graft-versus-host disease. Use cautiously in severe renal impairment or dialysis because evidence is lacking.

Monitoring
- Obtain a full blood count and liver-function tests before initiation.
- Monitor liver-function tests at least monthly during treatment.
- Assess promptly for hepatotoxicity, infection, cytopenias and other clinically significant adverse reactions.
Clinical pharmacology
Belumosudil is a selective ROCK2 inhibitor affecting immune and fibrotic signalling. Exposure increases with food. It is predominantly metabolised through CYP3A4, is highly protein-bound and is eliminated mainly in faeces.
Formulation and product differences
- The pale-yellow film-coated tablet must not be broken, crushed or chewed.
- It is essentially sodium-free.
- It is supplied in a moisture-protective bottle with a desiccant; retain the original packaging and close the cap securely.
