Prescribing considerations
- Restrict prescribing and administration to clinicians experienced in post-transplant immunosuppression, with resuscitation and laboratory support available.
- Confirm that graft implantation and accompanying immunosuppression will proceed before administration.
- Record the product name and batch number for biological traceability.
- Consider total immunosuppressive burden when assessing infection, opportunistic infection and lymphoproliferative risk.
- Avoid live attenuated vaccines; responses to inactivated vaccines may be reduced.
- Do not use for heart transplantation: efficacy and safety have not been established, and serious cardiac events were reported more frequently in small studies.

- Permanently discontinue after a severe hypersensitivity reaction; use particular caution with re-exposure after abandoned transplantation or early graft loss.

- Monitor graft function and laboratory parameters required by accompanying immunosuppressants; no basiliximab therapeutic drug monitoring is specified.

Clinical pharmacology
Basiliximab is a chimeric IgG1 monoclonal antibody that binds with high affinity to CD25 on activated T lymphocytes, preventing interleukin-2-mediated proliferation. It is not myelosuppressive, and conventional metabolic interactions are not expected.
Formulation and product differences
- Presentations differ in whether water for injections is supplied with the powder or obtained separately.
- After reconstitution, the solution should be clear to opalescent and free from visible particles.
- Do not mix with other intravenous medicines; administer through a separate infusion line.

Live attenuated vaccines
