Prescribing considerations
- Treatment should be supervised by a clinician experienced in Pompe disease or inherited metabolic or neuromuscular disorders.
- Consider preventive antihistamine, antipyretic or corticosteroid treatment according to previous infusion tolerance.
- Acute illness, advanced disease, fluid-overload susceptibility and compromised cardiac or respiratory function increase infusion risk.
- Home infusion requires documented tolerability, trained personnel, appropriate infrastructure and ready access to clinical support.
Contraindications and cautions
- Life-threatening hypersensitivity to avalglucosidase alfa or an excipient when attempted re-exposure was unsuccessful.
Monitoring
- Record the product name and batch number for biological traceability.
- Monitor vital signs and symptoms of hypersensitivity or infusion-associated reactions during administration and afterwards as clinically indicated.
- Ensure resuscitation and cardiorespiratory support are immediately available, particularly for patients with cardiac hypertrophy or substantial respiratory impairment.
- Consider anti-drug antibody testing when response is inadequate, and immunological testing when allergic risk or previous anaphylaxis warrants it.
- Monitor vulnerable patients for fluid overload and acute deterioration in cardiac or respiratory status.
Clinical pharmacology
Avalglucosidase alfa is recombinant human acid alpha-glucosidase modified with additional bis-mannose-6-phosphate structures. Receptor-mediated uptake delivers it to lysosomes, where proteolytic processing increases enzyme activity and promotes glycogen degradation.
Formulation and product differences
- Single-use powder for concentrate for intravenous infusion.
- Requires aseptic reconstitution, dilution in glucose solution and administration through a low-protein-binding in-line filter.
- Must not be mixed with other medicines or administered through the same intravenous line.

avalglucosidase alfa interactions
Formal interaction studies have not been performed. Cytochrome P450-mediated interactions are considered unlikely because avalglucosidase alfa is a recombinant human protein.
