Prescribing considerations
- Restricted to inpatient use under a clinician experienced in haemophilia treatment.
- Assess clinical haemostasis alongside factor VIII activity because recovery and half-life vary between patients.
- Test for anti-recombinant porcine factor VIII antibodies before treatment where practicable; treatment may begin before the result when clinically necessary.
- Record the product name and batch number for biological traceability.
- Consider cardiovascular and thromboembolic risk, especially in older patients and those with cardiovascular disease.
- Account for cumulative sodium exposure where many vials are required and note that polysorbate 80 may cause allergic reactions.
Contraindications and cautions
- Hypersensitivity to susoctocog alfa, hamster protein or any excipient.
- Congenital haemophilia A with inhibitors.
Monitoring
- Clinical bleeding status and haemodynamic condition.
- Plasma factor VIII activity using a consistent assay methodology; the one-stage clotting assay is recommended in the SmPC.
- Human and porcine factor VIII inhibitor titres where clinically relevant, particularly if response is inadequate.
- Hypersensitivity, thromboembolic complications and cardiovascular events.
Clinical pharmacology
Susoctocog alfa is a recombinant, B-domain-deleted, porcine-sequence factor VIII produced in baby hamster kidney cells. It binds von Willebrand factor and restores factor VIII cofactor activity within the intrinsic coagulation pathway; human factor VIII autoantibodies often have little or no cross-reactivity with it.
Formulation and product differences
- The selected product is supplied as a white powder with a pre-filled syringe of water for injections and a transfer device.
- The reconstituted solution contains sodium and polysorbate 80 and may contain trace hamster proteins.
- It is for intravenous use only and must not be mixed with other injectable medicines because compatibility studies are unavailable.
