Prescribing considerations
- Restricted to hospital use for adults requiring reversal of apixaban or rivaroxaban because of life-threatening or uncontrolled bleeding.
- Confirm the factor Xa inhibitor, last exposure details and baseline clinical situation where possible.
- Not suitable as pretreatment for urgent surgery and does not reverse non-factor Xa anticoagulants.
- Consider resuming appropriate anticoagulation once haemostasis is secure because the underlying thrombotic risk remains.
- Record the product name and batch number for biological traceability.
Contraindications and cautions
- Hypersensitivity to andexanet alfa or any formulation ingredient.
- Known allergy to hamster proteins.
- Exercise particular caution where thrombotic risk is high, including a history of stroke, myocardial infarction or heart failure.
- Avoid use before procedures requiring heparinisation because clinically important heparin unresponsiveness can occur.
Monitoring
- Assess clinical haemostasis, ongoing or recurrent bleeding and adverse events.
- Monitor closely for arterial and venous thrombotic events.
- Do not assess reversal using routine commercial anti-factor Xa activity assays after administration; they can produce misleadingly elevated results.
- Observe for infusion reactions and manage according to severity.
Clinical pharmacology
A recombinant, catalytically inactive factor Xa decoy that sequesters direct factor Xa inhibitors. It also binds and inhibits tissue factor pathway inhibitor, increasing tissue-factor-initiated thrombin generation.
Formulation and product differences
- The selected UK presentation is a refrigerated powder for solution for intravenous infusion.
- After reconstitution with water for injections, the solution is transferred without further dilution to a suitable syringe or infusion bag and administered through an appropriate low-protein-binding filter.
- It must not be mixed with other medicines because compatibility studies are unavailable.

andexanet alfa interactions
Formal interaction studies have not been performed. Important practical concerns include:
Unfractionated heparin
Andexanet alfa can neutralise heparin's anticoagulant effect and cause heparin unresponsiveness, so use before procedures requiring heparinisation should be avoided.
Low-molecular-weight heparins
Their anticoagulant activity may be temporarily affected; andexanet alfa has not been evaluated or recommended as their antidote.
Prothrombin complex concentrates, activated concentrates, recombinant factor VIIa, fresh frozen plasma or whole blood
Combined use has limited safety evidence and should be avoided unless clinically essential.
