Prescribing considerations
- Supervise treatment through a clinician experienced in Pompe disease or inherited metabolic or neuromuscular disorders.
- Assess clinical condition before each infusion; acute infection may increase infusion-reaction risk.
- Ensure resuscitation facilities and anaphylaxis treatment are immediately available.
- Record product name and batch number for biological traceability.
- Renal and hepatic impairment have not been specifically evaluated.
- Consider CRIM status and immunogenicity risk in infantile-onset disease.
Contraindications and cautions
- Life-threatening hypersensitivity to alglucosidase alfa or an excipient when rechallenge was unsuccessful.
- Use particular caution when considering re-administration after anaphylaxis, severe infusion reactions or systemic immune-mediated reactions.
- Advanced cardiac or respiratory disease may increase the consequences of an infusion-associated reaction.
Monitoring
- Monitor during and after infusion for immediate or delayed reactions.
- Evaluate response across motor, respiratory and cardiac manifestations as appropriate.
- Monitor IgG antibodies according to phenotype, response and safety concerns; consider IgE testing after hypersensitivity.
- Perform periodic urinalysis in patients with high IgG antibody titres.
Clinical pharmacology
A recombinant human acid alpha-glucosidase intended to restore lysosomal enzyme activity and promote glycogen degradation. Central nervous system uptake is unlikely because of molecular size and the blood–brain barrier.
Formulation and product differences
- White to off-white powder for concentrate for intravenous infusion.
- Requires aseptic reconstitution with water for injections and dilution in sodium chloride solution.
- Administer through a low-protein-binding in-line filter; do not mix with other medicinal products.
- Contains mannitol, phosphate salts and polysorbate 80 and is essentially sodium-free.

alglucosidase alfa interactions
Formal interaction studies have not been performed. Interactions through cytochrome P450 metabolism are unlikely, but the specialist team should review all medicines.
Immunosuppressive medicines
May further increase the risk of serious respiratory infection in people with Pompe disease and require additional vigilance.
