This is an implementation workflow for candidates who already have — or plan to buy — MRCEM Exam Prep and want a disciplined loop rather than another product ranking. It is built for busy trainees revising around clinical work. MRCEM SBA is 180 single-best-answer questions across two two-hour papers of 90, roughly 80 seconds per item, mapped to the 2021 RCEM curriculum (Year 1–3 SLOs). The weighting — Complex/Stable 55, Resuscitation 40, Injured 30, Paediatric EM 25, Procedural 20, Complex/Challenging 10 — is the target proportion for your practice. There is no negative marking, the pass mark is set by modified Angoff, and MRCEM Primary is a prerequisite. The RCEM sample questions and regulations pack are your format calibration; treat any vendor alignment claim as something to verify against that official material.

Step 1 — Blueprint inventory and a protected unseen pool

Before your first block, build the six-domain inventory from the weighting and set your target proportions. Then reserve a protected pool of questions you will not touch during learning — either a held-back segment or, better, a separate unseen bank — so you always have clean items to measure on. Without this, your end-of-revision percentage will measure memory of worked items, not readiness.

Step 2 — First pass: topic blocks only where foundations are weak

Use topic-filtered blocks only for domains where your baseline is genuinely weak; there, cueing helps you learn. Everywhere else, start mixed, because topic-filtered practice quietly tells you the answer's category ("this is a paediatrics block") in a way the real paper never will. The default is mixed; topic blocks are the exception you justify with a measured weakness.

Step 3 — Error review by code, not transcription

For every miss, assign a short error code — knowledge gap, misread stem, wrong risk trade-off, calculation slip, guideline out of date — and write one corrective action. Do not transcribe the whole explanation; that feels productive and changes nothing. The code lets you see your pattern (for example, mostly misread stems under time) and fix the cause rather than the instance.

Step 4 — Transfer practice before repetition

When you get an item wrong, do not immediately re-answer that same item. Instead answer a new item testing the same principle — a transfer test — before you ever see the original again. Step 5 — Switch to mixed timed blocks at the domain floor

Move to mixed, timed blocks as soon as your domain floors are met on unseen items — even if the first pass is incomplete. Completion is not the trigger; unseen performance is. Practise the 90-item, two-hour rhythm until pacing at roughly 80 seconds per item becomes automatic and holds to the end of the block.

Step 6 — Exit criteria (not 100% completion)

You are ready to stop new questions when every domain is at floor on unseen timed blocks; when your pace holds near 80 seconds per item to the end of a 90-item block; when high-confidence errors are rare; when spaced re-tests hold; and when your performance on the official RCEM sample material is solid. None of these is "finished the bank".

Worked example: seven days for a busy trainee

Days 1–2: mixed first-pass blocks (30–40 items around shifts), error-coded, on MRCEM Exam Prep, with topic blocks only for your single weakest domain. Days 3–4: repeat, widening to weighted proportions, and run transfer tests on iatroX for the concepts you missed. Day 5: a timed, 90-item-equivalent mixed block on unseen items to measure pace and domain floors. Day 6: review only the high-confidence errors and timing failures, and patch one domain with a tutorial. Day 7: rest, or one light official-sample block. The bank does breadth and learning; the unseen block issues the readiness verdict.

An error-code scheme you can reuse

The review loop only works if your codes are few and consistent. Use a small, fixed set so you can see patterns across a week rather than re-inventing categories each session.

At the end of the week, tally the codes. A week dominated by S and C codes is a pacing-and-technique problem that more content will not fix; a week dominated by K codes is a genuine knowledge phase where the tutorials earn their place.

Reading your weekly numbers

Judge the week by three numbers, not by items completed. The first is unseen accuracy by domain against the weighting — the only figure that certifies readiness. The second is your pace on a full-length timed block: holding roughly 80 seconds per item to the end matters more than raw accuracy on a leisurely set. The third is your high-confidence error rate, the items you were sure of and got wrong, which is the most dangerous category and the last to fall. If all three are moving in the right direction, the workflow is working regardless of how much of the bank remains.

A second micro-example: strong knowledge, poor pacing

Not every candidate needs more content. Take a trainee whose unseen accuracy is already at floor across domains but who cannot finish a 90-item block in time. The workflow changes shape: almost no new topic blocks, and instead daily mixed timed blocks that deliberately rehearse the 80-second decision, with review restricted to timing failures and high-confidence errors. The bank's role shrinks to supplying fresh items for the timed blocks, and the exit criterion is purely about pace — a full block completed within time with accuracy holding to the final ten items. Adding tutorials here would be comfort, not progress.

Three mistakes this workflow is designed to stop

The first is grinding to 100% completion as if the bank were the target; the target is unseen, timed performance, and a finished bank is neither necessary nor sufficient. The second is transcribing explanations instead of coding errors — it feels like work and changes nothing, whereas a one-line code plus one action changes behaviour. The third is measuring on items you have already seen; re-answering worked questions inflates your percentage and hides the gap, which is why the reserved unseen pool and the transfer step exist. Keep the loop tight: learn on the bank, code the error, test transfer on a fresh item, and certify on unseen timed blocks.

Decision checklist

  • Continue if: the first pass is incomplete and unseen accuracy is rising.
  • Supplement if: one modality (ECG, paediatric dosing) stays weak — add a targeted tool.
  • Switch if: content reads stale against current guidance, or you are re-seeing items.
  • Stop new questions if: the exit criteria are met — move to simulation and rest.

Judge on measured gaps, not novelty or sunk cost.

Is MRCEM Exam Prep enough for MRCEM SBA on its own? As the engine of a first pass and error-driven learning it can be your single primary bank, but the workflow only works if you bolt on the official RCEM sample material for calibration and an unseen pool for measurement. Used alone and re-scored, it certifies memory rather than readiness; those two additions are what make "enough" true.

Which MRCEM SBA component does MRCEM Exam Prep not reproduce well? The timed, whole-paper prioritisation of 90 items in two hours.
