Why do patients with severe hepatic impairment need close supervision when taking Femara (letrozole)?

Patients taking Femara (letrozole) with severe hepatic impairment (Child-Pugh C) need close supervision because systemic exposure and terminal half-life were approximately doubled compared with healthy volunteers.
