Prioritised differential diagnosis
- 1. Heart failure with systemic congestion, potentially precipitated or aggravated by recent NSAID exposure: raised JVP and dependent pitting oedema strongly favour this working diagnosis, and hypertension is a recognised risk factor NICE CKS. NSAID treatment has been associated with oedema, hypertension and cardiac failure, making ibuprofen a plausible contributor rather than a proven cause SmPC Nabumetone.
- 2. NSAID-associated oedema without established heart failure: drug-related oedema remains an alternative, although the raised JVP makes assessment for heart failure particularly important NICE CKS,SmPC Nabumetone.
- 3. Renal disease, including nephrotic syndrome, or hepatic disease with hypoalbuminaemia: these are recognised alternatives causing peripheral oedema NICE CKS.
- 4. Dependent oedema from venous insufficiency or prolonged inactivity: these remain possibilities, but rank below heart failure given the accompanying raised JVP NICE CKS.
Five discriminating history or examination findings
- 1. Orthopnoea or paroxysmal nocturnal dyspnoea: their presence would move heart failure further ahead of isolated drug-related or dependent oedema; their absence would reduce support but would not exclude heart failure because its presentation can be nonspecific NICE CKS.
- 2. Additional cardiac or pulmonary congestion signs: basal crepitations, pleural effusions, a displaced apex, gallop rhythm or a murmur would strengthen the heart-failure diagnosis; their absence would leave the competing causes more plausible NICE CKS.
- 3. Exact timing of swelling relative to ibuprofen and any other oedema-associated medicines: onset after NSAID exposure would increase suspicion of drug-associated oedema or NSAID-aggravated heart failure, whereas swelling predating exposure would weaken that explanation NICE CKS,SmPC Nabumetone.
- 4. A history of established renal or hepatic disease: either would raise renal or hepatic oedema, including hypoalbuminaemia, in the ranking; neither history alone would resolve the competing possibility of heart failure NICE CKS.
- 5. Prolonged inactivity or established venous insufficiency: either would raise dependent oedema in the differential, particularly if additional heart-failure features are absent, although the existing raised JVP continues to favour cardiac congestion NICE CKS.
Evidence boundary: the supplied excerpts do not establish a captopril–ibuprofen interaction causing acute kidney injury in this patient, so that specific mechanism cannot be confirmed from these sources NICE CKS,SmPC Nabumetone.
