What documentation and reporting procedures are required following a transfusion reaction?

Following a transfusion reaction, specific documentation and reporting procedures are required:

  • Document discussions in the patient's notes .
  • Provide the patient and their GP with copies of the discharge summary or other written communication that explains any adverse events related to the transfusion .
  • Provide verbal and written information to patients who have had a transfusion, and their family members or carers (as appropriate), explaining any adverse events .
