A tourniquet that has been in place for a prolonged period and is close to definitive care should be left undisturbed and the application time documented, because releasing or loosening it risks rebleeding and a reperfusion insult best managed in hospital. Intermittent loosening (B) is outdated and dangerous. Once committed and near hospital, leave it on and record the time.
