An unconscious hypoglycaemic patient without intravenous access is treated with intramuscular glucagon 1 mg, because oral glucose is unsafe owing to the aspiration risk and intravenous dextrose needs access not yet available. Oral gel (B) is contraindicated when consciousness is impaired. Glucagon is less effective in malnourished, alcohol-dependent or hepatically impaired patients, so intravenous dextrose remains preferable once access is obtained.
