Age over 50, proximal girdle pain, prolonged stiffness, high inflammatory markers and normal CK support polymyalgia rheumatica. Polymyositis would cause objective weakness and raised CK. Late-onset RA has synovitis, while fibromyalgia and rhabdomyolysis do not fit the inflammatory profile.
