An adnexal mass with an empty uterus and positive pregnancy test in a symptomatic woman is most consistent with ectopic pregnancy. A complete miscarriage may also leave an empty uterus, but the adnexal mass and unilateral pain make ectopic pregnancy the best answer. The pearl is that haemodynamic stability does not exclude ectopic pregnancy.
