Euvolaemic hypotonic hyponatraemia with concentrated urine and high urine sodium is SIADH. A hilar mass raises concern for small-cell lung cancer, a classic cause of ectopic ADH, and pneumonia can also contribute. Psychogenic polydipsia would suppress ADH and produce dilute urine.
