Infants with fever, poor feeding, bulging fontanelle and poor perfusion may have meningitis or sepsis and need urgent hospital assessment. Absence of rash does not exclude serious bacterial infection. Delaying until a rash appears risks deterioration. The pearl is that infants may present with non-specific signs before classic meningism.
