Correct answer: C — Repeat serum hCG measurements and arrange follow-up scanning

A positive pregnancy test with no pregnancy seen on transvaginal ultrasound is pregnancy of unknown location and needs serial hCG and follow-up until the location or resolution is clear. Reassurance is unsafe because ectopic pregnancy remains possible. Methotrexate is not given without meeting criteria and excluding viable intrauterine pregnancy. Cervical screening has no diagnostic role.
