Irregular bleeding is common during the first 3 to 6 months after LNG-IUS insertion. Absence of pain, discharge, pregnancy symptoms and visible threads makes serious pathology less likely, although safety-netting is needed. The bleeding pattern does not confirm PID or contraceptive failure. Persistent or new bleeding after an initial settling period should be assessed.
