Correct answer: EArrange urgent assessment for intravenous fluids and antiemetics

Weight loss, dehydration and ketonuria suggest hyperemesis gravidarum requiring urgent assessment and rehydration. Lifestyle measures may help mild nausea but are inadequate here. Oral iron can worsen nausea and does not address dehydration. Ultrasound may be used to assess viability or multiple pregnancy, but immediate clinical stabilisation comes first.
