Ovarian torsion is suggested by sudden severe unilateral pain, vomiting and a known ovarian cyst, and requires emergency gynaecological assessment. Delayed ultrasound risks ovarian necrosis. Cystitis does not explain guarding and vomiting with known cyst. A negative pregnancy test helps exclude ectopic pregnancy but does not exclude torsion.
