La sindrome retrovirale acuta (infezione primaria da HIV) si presenta 2-4 settimane dopo l'esposizione con febbre, linfoadenomegalia, rash, faringite e talvolta ulcere orali. Il test anticorpale può essere negativo (finestra sierologica) ma l'HIV-RNA è già rilevabile a titolo elevato. La diagnosi richiede test di 4ª generazione (Ag p24 + Ab) o HIV-RNA. L'inizio immediato della ART è raccomandato.
