MCUG is no longer routinely recommended after a first febrile UTI in children >6 months with a normal ultrasound. Current indications include: abnormal renal ultrasound (hydronephrosis, scarring, abnormal kidneys), recurrent febrile UTIs, male sex with febrile UTI (especially <6 months), family history of VUR, and atypical organisms. The decision is individualised.
