Unilateral testicular enlargement with elevated beta-hCG suggests an hCG-secreting testicular germ cell tumour (choriocarcinoma component). hCG stimulates testicular Leydig cells to produce testosterone, which is aromatised to oestradiol causing gynaecomastia. Urgent testicular ultrasound and oncology referral are required.
