In a young athlete with a family history of sudden cardiac death, a 12-lead ECG is the minimum recommended screening investigation in addition to history and physical examination. Echocardiography should also be strongly considered to exclude structural heart disease (HCM, ARVC). The Italian model of mandatory ECG screening has demonstrated significant reduction in SCD in athletes.
