Spastic diplegia (bilateral spastic CP, lower limbs > upper) is most commonly associated with prematurity and periventricular leukomalacia. The motor tracts to the lower limbs pass through the periventricular white matter and are selectively vulnerable to ischaemic injury. The scissoring gait pattern with relatively preserved upper limb function is characteristic.
