IV adenosine is the first-line pharmacological treatment for SVT after failed vagal manoeuvres. It should be given as a rapid bolus followed immediately by a normal saline flush. Initial dose is 100 mcg/kg (max 6 mg), increasing to 200 mcg/kg (max 12 mg) if unsuccessful.
