A 70-year-old man with advanced COPD and chronic hypoxaemia is being assessed for LTOT. His resting PaO₂ on room air is 56 mmHg on two measurements 3 weeks apart. He has no evidence of cor pulmonale or polycythaemia. Does he meet criteria for LTOT?

Correct answer: C — No – PaO₂ must be ≤55 mmHg or 55–59 mmHg with complications

LTOT criteria require PaO₂ ≤55 mmHg on two stable measurements ≥3 weeks apart, OR PaO₂ 56–59 mmHg with evidence of end-organ consequences (pulmonary hypertension, cor pulmonale, polycythaemia with Hct >55%, or peripheral oedema). This patient's PaO₂ of 56 mmHg falls in the 56–59 mmHg range but he has NO complications – therefore he does NOT currently meet criteria. He should be reassessed if complications develop or PaO₂ declines further. Ambulatory or nocturnal oxygen may be considered separately.
