Persistent microalbuminuria (confirmed on ≥2 of 3 samples) in T1DM indicates early diabetic nephropathy. ACE inhibitors are the first-line renoprotective treatment. They reduce intraglomerular pressure and proteinuria. BP target is <90th centile for age. Glycaemic optimisation is also essential.
