Unilateral MCDK with a normal contralateral kidney: conservative management with serial USS monitoring. The MCDK typically involutes over the first few years. VCUG is indicated if the contralateral kidney shows hydronephrosis (to exclude VUR). Nephrectomy is rarely needed unless the kidney is very large or causes symptoms.
