ADEM is a post-infectious inflammatory demyelinating disorder presenting with encephalopathy plus multifocal neurological deficits. MRI shows large, poorly-defined white matter lesions. Most children make a good recovery with IV methylprednisolone. If relapsing, MOG antibody disease or MS should be considered.
