Approximately 20–50% of children with HSP develop renal involvement (HSP nephritis), usually within the first 1–3 months. Risk factors include age >7 years and persistent purpura. Most cases are mild (microscopic haematuria +/- mild proteinuria) and resolve. Severe cases with nephrotic-range proteinuria or renal impairment may require biopsy and immunosuppressive treatment.
