Behçet disease with sight-threatening panuveitis/retinal vasculitis refractory to conventional immunosuppression warrants anti-TNF therapy. Adalimumab is the preferred anti-TNF for Behçet uveitis (VISUAL trials). Infliximab is an alternative for acute sight-threatening disease. Interferon-alpha is also effective but less commonly used due to side effects.
