Nitroglycerin 0.4 mg (400 μg) sublingual tablet or metered-dose spray is the standard initial dose for acute anginal chest pain. It should be administered with the patient in a seated or semi-reclined position. The dose may be repeated every 5 minutes for up to 3 doses. If pain persists after 3 doses (or worsens at any point), suspect acute MI and activate EMS (911). Nitroglycerin is contraindicated if systolic BP <90 mmHg or if the patient has taken a PDE5 inhibitor (sildenafil/tadalafil) within 24-48 hours (risk of severe hypotension).
