Tranexamic acid is an antifibrinolytic agent that competitively inhibits the activation of plasminogen to plasmin, thereby reducing fibrinolysis (breakdown of the fibrin clot). When used as a 5% mouthwash (10 mL swished for 2 minutes, 4 times daily for 2-7 days) after dental extraction in anticoagulated patients, it stabilises the clot locally without affecting systemic coagulation. It is an important adjunctive haemostatic measure in patients on warfarin, DOACs, or antiplatelet agents, reducing post-extraction bleeding significantly.
