Hemisection (root amputation of one root of a multi-rooted tooth) is indicated when one root is severely compromised (furcation involvement, root fracture, root perforation, external resorption, endodontic failure on one root) while the other root has adequate bone support and a good prognosis for retention. The compromised root is sectioned and removed; the remaining root is treated endodontically and restored (usually with a crown). It is an alternative to extraction that preserves a functional abutment.
