A mucous retention pseudocyst (antral polyp) appears on panoramic or periapical radiograph as a smooth, dome-shaped, non-corticated radiopacity arising from the floor or wall of the maxillary sinus. It is caused by accumulation of mucous beneath the sinus membrane (Schneiderian membrane) due to obstruction of a seromucinous gland duct. It is very common (incidental finding in up to 10% of panoramic radiographs), benign, and usually asymptomatic. No treatment is required unless it is large enough to obstruct sinus drainage.
