Koplik spots are pathognomonic for measles. They appear as small (1-3 mm) blue-white or grey-white papules on an erythematous base on the buccal mucosa opposite the premolars/molars, 1-2 days before the skin rash appears. They are the earliest diagnostic sign of measles. The spots are transient, disappearing shortly after the maculopapular skin rash develops. Recognition of Koplik spots allows early diagnosis and isolation of the patient before the rash appears.
