The Vazirani-Akinosi (closed-mouth/tuberosity) technique is specifically designed for patients who cannot open their mouth adequately for a conventional IAN block — such as those with trismus from infection, TMD, or fracture. The injection is performed with the mouth closed, the needle is inserted parallel to the maxillary occlusal plane at the level of the mucogingival junction of the maxillary molars, and directed posteriorly into the pterygomandibular space. It simultaneously anaesthetises the IAN, lingual nerve, and mylohyoid nerve.
