Fluctuance — a palpable wave-like sensation indicating liquid (pus) accumulation beneath the mucosa or skin — is the most reliable clinical sign indicating that a dental abscess has localised and is ready for incision and drainage (I&D). A firm, non-fluctuant swelling indicates cellulitis (diffuse infection in solid tissues) which requires antibiotic therapy and may not yet be amenable to I&D. I&D is the definitive treatment for a fluctuant abscess; antibiotics alone are insufficient without drainage.
