An avulsed tooth that has been stored in an appropriate medium (HBSS, milk, saline) should be gently rinsed with saline or the storage medium before replantation. The root surface should NEVER be scrubbed, handled aggressively, or treated with chemicals that could destroy remaining viable PDL cells. The goal is to preserve as many PDL cells as possible for optimal healing. If the tooth was stored dry for >60 minutes, the root surface should be treated with sodium fluoride (to slow replacement resorption) before replantation.
