Hyperparathyroidism causes generalised skeletal demineralisation including the jaws. Excess parathyroid hormone (PTH) increases osteoclastic bone resorption, causing diffuse osteoporosis, loss of lamina dura (an early and characteristic radiographic sign), ground-glass trabecular pattern, and brown tumours (central giant cell lesions). Primary hyperparathyroidism is caused by parathyroid adenoma; secondary is caused by chronic renal failure. Serum calcium is elevated and phosphate is decreased in primary hyperparathyroidism.
