MRI is the gold standard for imaging the TMJ soft tissues, particularly the articular disc (meniscus). It provides excellent soft tissue contrast without ionising radiation, allowing visualisation of disc position (normal, anterior displacement with/without reduction), disc morphology, joint effusion, condylar marrow changes, and surrounding soft tissues. MRI is performed in both closed-mouth and open-mouth positions to assess disc dynamics. CT is superior for bony changes but does not visualise the disc well.
