Perimolysis — erosion of the palatal surfaces of the maxillary anterior teeth — is a pathognomonic pattern for chronic vomiting (bulimia nervosa, anorexia nervosa-purging type). Gastric acid (pH 1-2) contacts the palatal enamel surfaces directly during vomiting. Other signs include bilateral parotid enlargement (sialadenosis), angular cheilitis, pharyngeal erythema, and calluses on the dorsum of the hand (Russell sign). GERD typically causes erosion on palatal surfaces too but less dramatic than bulimic erosion.
