Paget disease of bone (osteitis deformans) in its late (sclerotic) stage produces a characteristic cotton-wool or ground-glass radiopaque pattern due to disorganised mosite bone formation. The maxilla is affected more commonly than the mandible. Clinical features include progressive jaw enlargement, spacing of teeth, hypercementosis, and loss of lamina dura. Serum alkaline phosphatase is markedly elevated. Complications include secondary osteosarcoma (rare, <1%).
