In a 9-year-old, a multilocular radiolucency with mixed epithelial and ectomesenchymal components on biopsy (epithelium resembling ameloblastic epithelium within a primitive ectomesenchymal stroma resembling dental papilla) is characteristic of ameloblastic fibroma. It is a true mixed odontogenic tumour occurring predominantly in children and young adults (mean age ~15 years). Unlike ameloblastoma, which is primarily epithelial and rare in children, ameloblastic fibroma has a neoplastic mesenchymal component.
