Odontogenic keratocyst (OKC, now termed keratocystic odontogenic tumour by WHO 2005, reverted to OKC in WHO 2017) typically presents as a well-defined, unilocular or multilocular radiolucency with scalloped, corticated borders. It often extends along the length of the mandibular body without significant buccolingual expansion (grows anteroposteriorly within the medullary bone). It has a high recurrence rate (25-60%) if treated by simple enucleation. Association with Gorlin syndrome (multiple OKCs, basal cell carcinomas, skeletal anomalies) should be excluded.
