Nikolsky sign is the clinical finding where apparently normal-looking epithelium can be made to slide or separate by applying gentle lateral pressure with a finger. It is positive in pemphigus vulgaris (due to acantholysis — loss of intercellular adhesion between keratinocytes from autoantibodies against desmoglein 3 and/or desmoglein 1). It may also be positive in pemphigoid and other vesiculobullous diseases but is most classically associated with pemphigus vulgaris.
