Epinephrine (adrenaline) 0.3-0.5 mg IM in the mid-outer thigh (using 1:1,000 concentration) is the first-line treatment for anaphylaxis. It should be administered immediately upon recognition. Epinephrine reverses bronchospasm (β2), increases heart rate and cardiac output (β1), and causes peripheral vasoconstriction (α1). Antihistamines and corticosteroids are adjunctive but are NOT substitutes for epinephrine. The dose may be repeated every 5-15 minutes.
