A periapical granuloma (dental granuloma) consists of chronically inflamed granulation tissue at the apex of a non-vital tooth. It contains lymphocytes, plasma cells, macrophages, and may contain proliferating epithelial rests of Malassez. If these rests proliferate sufficiently to form a lumen, it becomes a radicular (periapical) cyst. Despite the name, it is not a true granuloma (which contains epithelioid histiocytes forming granulomas).
