NICE NG87 recommends reducing the ADHD medication dose and referring to a paediatric hypertension specialist when a child or young person has sustained resting tachycardia above 120 beats per minute on two occasions. Both readings meet that threshold. Continuing unchanged with routine monitoring is therefore inappropriate. Permanent cessation is not automatically required by this recommendation; management begins with dose reduction and specialist assessment. Switching directly to another stimulant could perpetuate cardiovascular adverse effects and should not precede assessment. Adding a beta-blocker to permit continuation at the same stimulant dose is not the NICE-recommended response and could mask the adverse effect without addressing its cause.
